Commemoration Gifts

A gift to the OTIS Foundation is a special way to commemorate a person or an occasion. All gifts over $2 are tax deductible. A receipt will be forwarded to all donors.

How to arrange special occasion donations

When celebrating Christmas, a birthday or a special occasion, supporters can ask guests to give a financial donation to The OTIS Foundation, instead of a gift.

Download the “Celebration Gift Form” below and include with the party invitations sent to your guests.
Your guests can then complete the form and send it to The OTIS Foundation along with their donation.

The OTIS Foundation tallies all donations and provides you with a list of names and the total amount of the donations received (for privacy reasons we do not disclose individual donations amounts).
Tax-deductible receipts are issued directly to the donor (for all donations over $2).

Organising a donation collection at a funeral

You can ask friends to give a gift to The OTIS Foundation instead of sending flowers. If you are organising a funeral, ask the Funeral Director to provide you with OTIS Foundation in Memory donation envelopes.

If they are unable to help you, please ask the Funeral Director to contact The OTIS Foundation on (03) 5444 1185 or 0488 702224.

All gifts are acknowledged. A receipt will be forwarded to the donor and a list of donors' names and addresses will be provided to the family.

Collected donations can be delivered to our office, posted or collected by an OTIS staff member.
